2. Diagnostic tests using the EOPYY referral without charge for the participation of the insured, or with a 30% discount on the price list if done privately.
3. 1 free visit (by appointment) to the regular Outpatient Clinics of all main specialties* and a free full check-up which will include:
__ Complete laboratory blood test
__ Electrocardiogram/triplex of the heart
__Upper-lower abdomen and thyroid ultrasound
__ Chest X-ray
__ Spirometry
4. Contact line via email care@lefkosstavros.gr and answers to clinical questions.
All of the above benefits are valid for one year. In case of a second hospitalisation, the benefits will be renewed for 2 years.

1. Submitting and signing in physical or electronic form an application for the issuance and provision of the health card means that the applicant knows and unconditionally accepts the following terms and conditions of use and agrees that these regulate his/her rights and obligations.
2. The health card has been created for the purpose of providing discounted services, provided that the card is presented, which must be valid and in accordance with the rules and conditions of operation of the clinic and its general financial policy.
3. The amount of the discount and the type of medical examination or hospitalization, for which the holder will have a discount, will be determined by price lists drawn up by the clinic depending on its services, which are listed in the attached brochure.
4. The start and end date of the card’s validity is determined by the date of completion of the relevant application. This card will include the full name of the member and the duration of the card. After the expiry of the card’s validity, it can be renewed by submitting an application.
5. Each health card is personal and non-transferable.
6. A prerequisite for obtaining it is the registration of the non-sensitive personal data of the registrant, such as name, address, telephone number, etc. The personal data of the member will be recorded in a file which can be used exclusively for the needs of communication with the member, within the purpose of issuing the card and in accordance with the law. Under no circumstances shall the member’s data be made public or used in any way contrary to the member’s will. The Member shall allow the clinic to keep this record, which shall not include sensitive personal data, but only the general data of all holders.
7. The legitimate holder of the membership card will use his/her name card by presenting a document with it in order to facilitate his/her identification.
8. In case of loss, theft or destruction of the card, the member must immediately notify the clinic in any way. If the loss is not reported, the clinic is not liable in case of use of the card by a third party. After the declaration of loss, the card cancellation procedure follows.
9. The issuer reserves the right to cancel the card at any time if, in its sole discretion, the card is not used in accordance with the terms of participation or is used in a manner contrary to the purpose for which it was issued.
10. The card is provided without any subscription fee or any other cost.
11. For any dispute that may arise, the courts of Athens shall have jurisdiction.
